Box 3912–40100, Kisumu in the Republic of Kenya, formerly known as Hellen Hazel Miseda, formally and absolutely renounced and abandoned the use of her former names Hellen Hazel Miseda, and in lieu thereof assumed and adopted the name Hazel Hellen Miseda Mumbo, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Hazel Hellen Miseda Mumbo only.
