Box 54029–00200, Nairobi in the Republic of Kenya, on behalf of Raymond Ashwini Mosoba (minor), formerly known as Raymond Karuma Mosoba, formally and absolutely renounced and abandoned the use of his former name Raymond Karuma Mosoba and in lieu thereof assumed and adopted the name Raymond Ashwini Mosoba, for all purposes and authorizes and requests all persons at all times to designate, describe and address him by his assumed name Raymond Ashwini Mosoba only.
