Folio 50/512, File No. MMXXII, by our client, Philip Munyao Mutisya Munyambu, of P.O. Box 35597-00100, Nairobi in the Republic of Kenya, formerly known as Philip Munyao Mutisya, formally and absolutely renounced and abandoned the use of his former name Philip Munyao Mutisya, and in lieu thereof assumed and adopted the name Philip Munyao Mutisya Munyambu, for all purposes and authorizes and requests all persons at all times to designate, describe and address him by his assumed name Philip Munyao Mutusya Munyambu only.
