Box 6635-00100, Nairobi in the Republic of Kenya, formerly known as Ivy Wambui Macharia, formally and absolutely renounced and abandoned the use of her former name Ivy Wambui Macharia, and in lieu thereof assumed and adopted the name [vy Maryam Wambui Macharia, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Ivy Maryam Wambui Macharia only.
