Box 98287–80100, Mombasa in the Republic of Kenya, on behalf of Mercy Bahati (minor), formerly known as Mercy Nyambura, formally and absolutely renounced and abandoned the use of her former name Mercy Nyambura, and in lieu thereof assumed and adopted the name Mercy Bahati for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Mercy Bahati only.
