Box 1890–90100, Machakos in the Republic of Kenya, formerly known as Jossylyn Nzilani Mutua, formally and absolutely renounced and abandoned the use of her former name Jossylyn Nzilani Mutua and in lieu thereof assumed and adopted the name Dr. Jossylyn Nzilani Mutua, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Dr. Jossylyn Nzilani Mutua only.
