Box 86417–80100, Mombasa in the Republic of Kenya, formerly known as Ngunzi Godfrey Mwatu, formally and absolutely renounced and abandoned the use of my former name Ngunzi Godfrey Mwatu, and in lieu thereof assumed and adopted the name Mwatu wa Ngunzi, for all purposes and authorizes and requests all persons at all times to designate, describe and address me by my assumed name Mwatu wa Ngunzi only. MWATU WA NGUNZI, MR/5175743 formerly known as Ngunzi Godfrey Mwatu.
