Box 2101, Mombasa in the Republic of Kenya, formerly known as Mildred Joan Aoko Okello, formally and absolutely renounced and abandoned the use of her former name Mildred Joan Aoko Okello and in lieu thereof assumed and adopted the name Mildred Aoko Okello, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Mildred Aoko Okello only.
