Box 306–00204, Athi River in Kenya, formerly known as Stacey Grace Wanjiru Maingey, formally and absolutely renounced and abandoned the use of her former name Stacey Grace Wanjiru Maingey and in lieu thereof assumed and adopted the name Kisela Gracee Dezirae Esmeralda Hillman, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Kisela Gracee Dezirae Esmeralda Hillman only.
