Box 25–10303, Wang’uru in the Republic of Kenya, formerly known as Veronica Boke John, formally and absolutely renounced and abandoned the use of her former name Veronica Boke John and in lieu thereof assumed and adopted the name Kelly Boke John, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Kelly Boke John only.
