Box 60300– 00300, Isiolo in the Republic of Kenya, formerly known as Mumina Wako Happi Wario, formally and absolutely renounced and abandoned the use of her former name Mumina Wako Happi Wario and in lieu thereof assumed and adopted the name Mumina Wako Happi, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Mumina Wako Happi only.
