Box 434–80400, Ukunda in the Republic of Kenya, formerly known as Juliet Njeri Waithira, formally and absolutely renounced and abandoned the use of my former name Juliet Njeri Waithira and in lieu thereof assumed and adopted the name Juliet Njeri Kariuki, for all purposes and authorizes and requests all persons at all times to designate, describe and address me by my assumed name Juliet Njeri Kariuki only.
