Box 81615–80100, Mombasa in the Republic of Kenya, on behalf of Fatma Swaleh Mbarak Salim (minor), formerly known as Fatma Said Said, formally and absolutely renounced and abandoned the use of her former name Fatma Said Said and in lieu thereof assumed and adopted the name Fatma Swaleh Mbarak Salim, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Fatma Swaleh Mbarak Salim only.
