Box 721– 80109, Mombasa in the Republic of Kenya, formerly known as Emily Muthoni, formally and absolutely renounced and abandoned the use of her former name Emily Muthoni and in lieu thereof assumed and adopted the name Emily Muthoni Kangethe, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Emily Muthoni Kangethe only.
