Box 56961-00200, Nairobi in the Republic of Kenya, formerly known as Esther Yvonne Njambi Chege, formally and absolutely renounced and abandoned the use of her former name Esther Yvonne Njambi Chege, and in lieu thereof assumed and adopted the name Yvonne Njambi Chege, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Yvonne Njambi Chege only.
