Box 41853–00100, Nairobi in Kenya, formerly known as Crista Victoria Jappinen, formally and absolutely renounced and abandoned the use of her former name Crista Victoria Jappinen and in lieu thereof assumed and adopted the name Crista Victoria Mehta, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Crista Victoria Mehta only.
