Box 83145–80100, Mombasa in the Republic of Kenya, formerly known as Sharlet Dzine Mwamulanda, formally and absolutely renounced and abandoned the use of my former name as Sharlet Dzine Mwamulanda and in lieu thereof assumed and adopted the name Sharlet Mwamulanda, for all purposes and authorizes and requests all persons at all times to designate, describe and address me by my assumed name Sharlet Mwamulanda only.
