Box 2949- 40100, Kisumu in the Republic of Kenya, fonnedy known as Paul Vincent Njeli, formally and absolutely renounced and abandoned the use of his former name Paul Vincent Njeli, and in lieu thereof assumed and adopted the name Njeli Paul Vincent Opole, for all purposes and authorizes and requests all persons at all times to designate, describe and address him by his assumed name Njeli Paul Vincent Opole only.
