Box 81070-80100, Mombasa in the Republic of Kenya, on behalf of Alina Sayyid (minor), formerly known as Alina Sharma, formally and absolutely renounced and abandoned the use of her former name Alina Sharma, and in lieu thereof assumed and adopted the name Alina Sayyid, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Alina Sayyid only.
