Box 5431-80100, Mombasa in the Republic of Kenya, formerly known as Chris Simeon Samora Kikuyu, formally and absolutely renounced and abandoned the use of my former name Chris Simeon Samora Kikuyu, and in lieu thereof assumed and adopted the name Chris Samora Kikuyu, for all purposes and authorizes and requests all perslins at all times to designate, describe and address me by my assumed name Chris Samora Kikuyu only.
