Box 20277–00100, Nairobi in the Republic of Kenya, formerly known as Damaris Wanjiku Njeri, formally and absolutely renounced and abandoned the use of my former name Damaris Wanjiku Njeri, and in lieu thereof assumed and adopted the name Damaris Wanjiku Duckworth, for all purposes and authorizes and requests all persons at all times to designate, describe and address me by my assumed name Damaris Wanjiku Duckworth only.
