Box 1972–30209, Kitale in the Republic of Kenya, formerly known as Philip Simiyu Natembea, formally and absolutely renounced and abandoned the use of my former name Philip Simiyu Natembea and in lieu thereof assumed and adopted the name of Philip Barasa Masindano, for all purposes and authorizes and requests all persons at all times to designate, describe and address me by my assumed name Philip Barasa Masindano only.
