Box 3400-00200, Nairobi in the Republic of Kenya, on behalf of Jebet Kimoi Kipkosgei (minor), formerly known as Lynne Jebet Kipkosgei, formally and absolutely renounced and abandoned the use of her former name Lynne Jebet Kipkosgei and in lieu thereof assumed and adopted the name of Jebet Kimoi Kipkosgei for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Jebet Kimoi Kipkosgei only.
