Box 1450-00900, Kiambu in the Republic of Kenya, formerly known as Lucy Njoki Wambui, formally and absolutely renounced and abandoned the use of my former name Lucy Njoki Wambui, and in lieu thereof assumed and adopted the name Lucy Njoki Ndinguri, for all purposes and authorizes and requests all persons at all times to designate, describe and address me by my assumed name Lucy Njoki Ndinguri only.
