Box 67948-00200, Nairobi in the Republic of Kenya, on behalf of Fortune Mutiso Mwinzi (minor) formerly known as Fortune Malenge Ulwenya, formally and absolutely renounced and abandoned the use of his former name Fortune Malenge Ulwenya and in lieu thereof assumed and adopted the name Fortune Mutiso Mwinzi, for all purposes and authorizes and requests all persons at all times to designate, describe and address him by his assumed name Fortune Mutiso Mwinzi only.
