Box 26310–00100, Nairobi in Kenya, formerly known as Eunice Bochaberi Omariba, formally and absolutely renounced and abandoned the use of her former name Eunice Bochaberi Omariba, and in lieu thereof assumed and adopted the name Kay Bochaberi Omariba, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Kay Bochaberi Omariba only.
