Box 80965–80100, Mombasa in the Republic of Kenya, formerly known as Jessie Bakhita Lupe, formally and absolutely renounced and abandoned the use of my former name Jessie Bakhita Lupe and in lieu thereof assumed and adopted the name Jessie Bakhita Flansburgh, for all purposes and authorizes and requests all persons at all times to designate, describe and address me by my assumed name Jessie Bakhita Flansburgh only.
