Box 3924-00200, Nairobi in the Republic of Kenya on behalf of Victiuos Wekesa Hamam (minor), formerly known as Victious Wekesa Hamam Wabululu, formally and absolutely renounced and abandoned the use of his former name Victious Wekesa Hamam Wabululu and in lieu thereof assumed and adopted the name Victiuos Wekesa Hamam, for all purposes and authorizes and requests all persons at all times to designate, describe and address him by his assumed name Victiuos Wekesa Hamam only.
