Box 26339-00100, Nairobi in the Republic of Kenya, formerly known as Gladwire Flora Ogino Shida, formally and absolutely renounced and abandoned the use of her former name Gladwire Flora Ogino Shida and in lieu thereof assumed and adopted the name Gladwell Flora Mauyo, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Gladwell Flora Mauyo only.
