Box 858-00232, Ruiru in the Republic of Kenya, formerly known as Edwin Kamau Kariuki, formally and absolutely renounced and abandoned the use of my former name Edwin Kamau Kariuki and in lieu thereof assumed and adopted the name Kamau Kariuki, for all purposes and authorizes and requests all persons at all times to designate, describe and address me by my assumed name Kamau Kariuki only.
