Box 90294- 80100, Mombasa, shall dispose of and destroy the following records in its possesion: In-patient files 2011 and 2012 Out-patient registers 2011 and 2012 Copies of outpatient and inpatient 2011 and 2012 radiology reports Copies of outpatient and inpatient 2011 and 2012 laboratory reports X-ray films 2011 and 2012 Reporting books 2011 and 2012 Burnt out inpatient files by recent fire 2018 Letters of undertaking 2011 and 2012 Invoice copies 2011 and 2012 Accordingly, take notice that any such records that no claim is made of and substantiated in accordance with the law, shall be destroyed without any further notice thereof.
