Box 7640-00100, Nairobi in the Republic of Kenya, formally known as Alencia Wavua Mkunda, formally and absolutely renounced and abandoned the use of her former name Alencia Wavua Mkunda and in lieu thereof assumed and adopted the name Allencia Wavua Mkunda for all purposes and authorizes and requests all persons at all times to designate describe and address her by her assumed name Allencia Wavua Mkunda only.
