Box 111, Ruiru in the Republic of Kenya, formerly known as Monica Wanjira Kamuchu, formally and absolutely renounced and abandoned the use of my former name Monica Wanjira Kamuchu,and in lieu thereof assumed and adopted the name Monica Wanjira Magondu, for all purposes and authorizes and requests all persons at all times to designate, describe and address me by my assumed name Monica Wanjira Magondu only.
