Box 9191- 30100, Eldoret in the Republic of Kenya, formerly known as Benard Kiplimo Langat, formally and absolutely renounced and abandoned the use of my former name Benard Kiplimo Langat and in lieu thereof assumed and adopted the name Bernard Kiplimo Araap Lagat for all purposes and authorizes and requests all persons at all times to designate, describe and address me by my assumed name Bernard Kiplimo Araap Lagat only.
