Box 258- 40400, Migori in the Republic of Kenya, formerly known as Purity Adhiambo Owuor, formally and absolutely renounced and abandoned the use of her former name Purity Adhiambo Owuor and in lieu thereof assumed and adopted the name Purity Adhiambo Otieno, for all purposes and authorizes and requests all persons at all times to designate describe and address her by her assumed name Purity Adhiambo Otieno only.
