Box 44–90100, Machakos in the Republic of Kenya, formerly known as Dorothy Elijah Maundu, formally and absolutely renounced and abandoned the use of my former name Dorothy Elijah Maundu and in lieu thereof assumed and adopted the name Dorothy Mutete, for all purposes and authorizes and requests all persons at all times to designate, describe and address me by my assumed name Dorothy Mutete only.
