Box 1667- 50100, Kakamega, formerly known as Kohe Chitahi Khasiani, formally and absolutely renounced and abandoned the use of his former name Kohd Chitahi Khasiani and in lieu thereof assumed and adopted the name Kohe Dukakis Alexandre, for all purposes and authorizes and requests all persons at all times to designate, describe and address him by his assumed name Kohe Dukakis Alexandre only.
