Box 1614- 00600, Nairobi in the Republic of Kenya, formerly known as Paritha Mayur Shah, formally and absolutely renounced and abandoned the use of her former name Paritha Mayur Shah and in lieu thereof assumed and adopted the name Paritha Adarsh Shah, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Paritha Adarsh Shah only.
