Box 4499- 00100, Nairobi, formerly known as Peninah Njeri ,Ndombi alias Peninah Njeri Thande, formally and absolutely renounced and abandoned the use of her former name' Peninah Njeri Ndombi alias Peninah Njeri Thande, and in lieu thereof ,assumed and adopted the name Peninah Njeri Wambu, for, all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Peninah Njeri Wambu only, Dated the 5th June', 2017.
