Box 1944– 00661, Nairobi, formerly known as Beatrice Njambi Mathu Wainaina, formally and absolutely renounced and abandoned the use of her former name Beatrice Njambi Mathu Wainaina, and in lieu thereof assumed and adopted the name Njambi Mathu Wainaina, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Njambi Mathu Wainaina only.
