Box 40532-80100, Mombasa in the Republic of Kenya, formerly known as Lisa Aoko, formally and absolutely renounced and abandoned the use of her former name Lisa Aoko and in lieu thereof assumed and adopted the name Lisa Njeri Mbote, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Lisa Njeri Mbote only.
