Box 3112-40100, Kisumu in the Republic of Kenya, formerly known as Sharon Adhiambo Opiyo, formally and absolutely renounced and abandoned the use of my former name Sharon Adhiambo Opiyo and in lieu thereof assumed and adopted the name Sharon Adhiambo Ouko, for all purposes and authorizes and requests all persons at all times to designate, describe and address me by my assumed name Sharon Adhiambo Ouko only.
