Box 16872, Nakuru in the Republic of Kenya. formerly known as Lisa Marya Bochaberi, formally and absolutely renounced and abandoned the use of her former name Lisa Marya Bochaberi and in lieu thereof assumed and adopted the name Lisa Marya Ford, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Lisa Marya Ford only.
