Box 20320-36, Nyahururu, formerly known as Kelly Wanjira Mwangi, formally and absolutely renounced and abandoned the use of her former name Kelly Wanjira Mwangi and in lieu thereof assumed and adopted the name Kelly Nyokabi Wairagu, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Kelly Nyokabi Wairagu only.
