Box 6, Doyers St. Ste 130321, New York, NY 10013, a resident of New York, formerly known as John Moshe Dayan Agollah, formally and absolutely renounced and abandoned the use of his former name John Moshe Dayan Agollah and in lieu thereof assumed and adopted the name Moshe Agola Dayan, for all purposes and authorizes and requests all persons at all times to designate, describe and address him by his assumed name Moshe Agola Dayan only.
