Box 1279-80100, Mombasa in the Republic of Kenya, on behalf of Pauline Adhiambo Allen formerly known as Pauline Adhiambo Akello, formally and absolutely renounced and abandoned the use of her former name Pauline Adhiambo Akello, and in lieu thereof assumed and adopted the name Pauline Adhiambo Allen, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Pauline Adhiambo Allen only.
