Box 16733- 20100, Nakuru in the Republic of Kenya, formerly known as Chesire Jemutai, formally and absolutely renounced and abandoned the use of her former name Chesire Jemutai and in lieu thereof assumed and adopted the name Sheila Jemutai Chesire, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Sheila Jemutai Chesire only.
