Box 8444– 00100, Nairobi in the Republic of Kenya, formerly known as Lena Salma Anunda, formally and absolutely renounced and abandoned the use of her former name Lena Salma Anunda, and in lieu thereof assumed and adopted the name Lena Salma Samuel, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Lena Salma Samuel only.
