Box 536–20115, Nakuru in the Reoublic of Kenya, on behalf of Myra Natalie Busolo (minor), formerly known as Myra Natalie Kironji, formally and absolutely renounced and abandoned the use of her former name Myra Natalie Kironji, and in lieu thereof assumed and adopted the name Myra Natalie Busolo, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Myra Natalie Busolo only.
