Box 61197–00200, Nairobi in the Republic of Kenya, on behalf of Maria Mukasia Siteyi (minor), formerly known as Maria Immaculate Mukasia Siteyi, formally and absolutely renounced and abandoned the use of her former name Maria Immaculate Mukasia Siteyi, and in lieu thereof assumed and adopted the name Maria Mukasia Siteyi, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Maria Mukasia Siteyi only.
